Today, I am going to talk about the connection between two medications that target different systems but often show up together in real-life treatment: Mounjaro and Cialis.
Are they safe to combine?

Or is this a mix we should approach with caution?
Let’s take a closer look at this because this is more than just a drug comparison. It’s about how we can approach metabolic health, weight loss, and sexual function more holistically.
Let me start with Mounjaro, also known as Tirzepatide, which is a once-weekly injection used for managing type 2 diabetes and also approved for weight loss. It works by activating two receptors: GLP-1 and GIP. These are hormones that regulate blood sugar, reduce appetite, slow gastric emptying, and promote weight loss.
Cialis, or tadalafil, belongs to a class of medications called PDE5 inhibitors. I prescribe it primarily for erectile dysfunction, but it’s also used for benign prostatic hyperplasia and pulmonary arterial hypertension. It works by increasing blood flow to specific areas of the body through vasodilation.

Based on the drug interaction, there is no direct contraindication or significant drug-drug interaction between tirzepatide and tadalafil. They work through completely different mechanisms, and we are metabolised through different pathways.

Cialis can cause mild hypertension due to its vasodilatory effects, while Mounjaro typically has neutral to slightly beneficial effects on cardiovascular parameters.
Many people who use Mounjaro may also benefit from Cialis, and vice versa, because Obesity, type 2 diabetes, and erectile dysfunction often coexist. Tirzepatide improves insulin sensitivity and reduces inflammation, both of which can naturally improve sexual function over time. Weight loss itself is associated with better testosterone levels and improved vascular function.
On the other hand, Cialis provides immediate support for ED, while also offering secondary benefits like improving lower urinary tract symptoms in men.

If you are on both medications, you should be aware of these things:
- Dizziness or lightheadedness, especially when standing up quickly. This could indicate low blood pressure.
- Gastrointestinal side effects like nausea or delayed stomach emptying caused by Mounjaro which might slightly affect how oral medications like Cialis are absorbed.
- Cialis can be taken as needed or daily. If using it as needed, it is better to avoid taking it at the exact time as your Mounjaro dose to reduce the overlap of side effects.
So, to answer the question, do they go hand in hand?
Yes, they can complement each other. Mounjaro addresses the root causes of metabolic dysfunction, such as weight, insulin resistance, and inflammation. Cialis offers symptomatic relief of erectile dysfunction and urinary symptoms.
Together, they can form a powerful approach to improving both metabolic health and quality of life in men dealing with diabetes, obesity, and ED.
